Talus Fracture - StatPearls - NCBI Bookshelf.

Abstract. Osteochondral lesions (OCL) of the talus involve both articular cartilage and subchondral bone of the talar dome. This term refers to a wide spectrum of pathologies including mild bone marrow contusion as well as severe osteoarthritis resulting from long standing disease.

An osteochondral lesion of the distal tibia and fibula has been thought of as a rare condition, although it is located in the same weight-bearing joint as an osteochondral lesion of the talus (OLT). However, our experience with ankle or foot MRI suggests a coexisting osteochondral lesion of the distal tibia and fibula is not rare in patients with an OLT.

Fractures of the Ankle Joint (23.05.2014).

The talus ranks as the second largest tarsal bone with unique anatomic features and serves an important role in mobility of the foot and ankle as it is the osseous link between the leg and the foot. The anatomy of the talus is quite complex with three separate articulations with distinct features including the subtalar joint with its three faces. The talus divides into a head, neck, and body. The MRI findings of unstable and stable osteochondritis dissecans of the capitellum are similar to the findings described for osteochondritis dissecans of the femoral condyles and talar dome. Unstable osteochondritis dissecans lesions are surrounded by a rim of high signal intensity or a fluid-filled cyst on T2-weighted images.Diagnosis: Avascular necrosis of talus. T2 weighted images display a typical double line sign which consisting of a hypointense outer line (sclerosis and fibrosis) and a hyperintense inner line (granulation tissue). No fracture could be identifie.

Osteochondral fractures of the dome of the talus which occur in association with ankle sprains are commonly overlooked. These fractures may occur when there is a compressive component to the inversion injury, especially with landing from a jump. The dome of the talus is compressed by the tibial plafond causing damage of the osteochondral surface.

Magnetic resonance imaging demonstrates the meniscal mass, chondral or osteochondral lesions, ligament thickening, besides osteophytes (1,3) (Figure 4), and conventional radiography demonstrates osteophytes, but, considering the uncommon nature of this condition, no extensive study reporting such imaging findings is available in the literature (4).

A third group of patients were identified with no history of injury to the syndesmosis or lateral ligament complex (NI group). These patients underwent MRI scanning for unrelated conditions such as osteochondral lesion of the talus or Achilles tendon injury. Identifying features were removed from the scans, which were then randomly listed for.
